Form 1583 authorizes a commercial mail receiving agency to receive mail for you. It does not establish address acceptance for a bank or state filing.

Start with the current form
Use the official USPS form or the current version supplied through your chosen provider's verified process. Match the mailbox address, private mailbox number, applicant details, and intended service type to the provider's instructions. Do not guess which address goes in a field.
Prepare your information
- Your applicant name, contact information, and home-address details.
- The CMRA's address and your assigned private mailbox details.
- Business information when applying for business use.
- Any authorized person who will collect mail.
- Forwarding information when the applicable section requires it.
- Accepted photo identification and address evidence from the current form's lists.
Verify documents and signatures
Check validity, matching names and addresses, and any exception with the provider before signing. Follow its witnessing or notarization process and ask about the cost. Do not assume a remote notarization service is included. The official instructions describe who may witness the applicant's signature.
Avoid common delays
Missing business fields, an address that does not match supporting evidence, an unsupported document, or an incorrect signature process can require correction. Ask how additional adult recipients and business names are handled rather than assuming one authorization covers everyone.
Submit through the provider
Use the provider's secure verification channel. HomeBase does not collect identity documents or submit this form. Keep a copy and ask when mail receipt can begin, what happens while verification is pending, and how to update changed or expired information.
Before signup, review verification and other mailbox costs and your provider shortlist.
